5/1/2023 EnchantaVaughn Approved 1.Behavior: Upon arrival, Jahsim appeared to be in a pleasant mood evident by greeting QMHP-C with a smile and saying, "Hello". Jahsim was receptive to the session evident by cooperating in the activities and sharing his thoughts and feelings. Jahsim was redirected a few times throughout today's session. 2. Behavior: Jahsim presented normal behaviors within the remainder of the session needing no redirection from QMHP-C for negative behavior presented but continued to encourage him to follow through with all directives gien to his mother. QMHP-C inquired with Jahsim about how his day was going and what it consisted of. QMHP-C actively listened as Jahsim shared what his day was like and monitored his ability to maintain eye contact throughout the discussion. QMHP-C processed with Jahsim how difficult it was to hear him clearly and identify who he was speaking to because he failed to make eye contact. QMHP-C engaged Jahsim to into a staring contest. QMHP-C explained to Jahsim that whoever was to break eye contact would be required to do 15 push ups. QMHP-C praised Jahsim for the positive eye contact demonstrated and encouraged him to stay focus for the negative eye contact demonstrated. 2. Intervention: QMHP-C asked probing questions to learn whether Jahsim had experienced any triggers of anger throughout the weekend, requiring him to share how he responded. QMHP-C processed with Jahsim some possible ways he could have responded to the triggers to keep things from escalating. QMHP-C challenged Jahsim to verbalize what the behaviors mentioned by QMHP-C would look like. QMHP-C praised Jahsim for his appropriate responses. QMHP-C engaged Jahsim into watching two short films on the importance of identifying triggers. 1.Response: Jahsim stated no changes needed to be made to safety plan.. Jahsim described his day to be going "good". Jahsim explained to QMHP-C that he did not go to school today because he was not feeling well. He shared that he was feeling a little better but was not sure if he would be attending school tomorrow. Jahsim showed moderate eye contact throughout the discussion evident by periodically making eye contact and frequently directing his attention to those around him. Jahsim was receptive to the processing evident by paying attention and nodding his head up and down. Jahsim was receptive to the activity evident by practicing positive eye contact and engaging in the activity. Jahsim was attentive as QMHP-C explained what was expected of him. Jahsim showed positive eye contact but was required to do pushups two times throughout the activity. 2. Response: Jahsim denied experiencing any triggers of anger over the weekend. He then was able to admit that he was angered by his sister and cousins when they turned his game off when he was not in the room. He shared that he became upset when he was told he could not go to a friends house by his grandmother. Jahsim was receptive to the processing evident by staying on task and agreeing with QMHP-C. Jahsim was able to voice what the behaviors would look like that were mentioned by QMHP-C. Jahsim was receptive to the videos evident paying attention and staying on task.
